When this lipid matrix is intact, the skin can retain moisture effectively and defend itself against irritants, allergens, and pollutants.

When the skin barrier becomes compromised, these protective lipids are depleted or disrupted. This leads to increased water loss and allows external irritants to penetrate the skin more easily. As a result, the skin may feel tight, dry, dehydrated, sensitive, reactive, or suddenly prone to breakouts. The ingredients needed to repair and rebuild the skin barrier are very different from those that simply mask the symptoms of a compromised barrier. While some products can temporarily smooth or soften the skin, true barrier repair requires replacing what the skin has lost.

The most important place to start is with the lipids that sit between the skin cells. A decline in these essential lipids is what allows water to escape from the skin and enables allergens, pollutants, and irritants to penetrate more easily. This is why a compromised skin barrier often presents as dehydration, sensitivity, inflammation, or breakouts.

Products that combine ceramides, cholesterol, and fatty acids help to mimic the skin’s natural barrier composition. These three lipids are the core components of a healthy skin barrier and are widely recognised as the gold standard for effective barrier repair.

Ceramides
Ceramides are the primary building blocks of the skin barrier and make up around 50% of the intercellular lipids found within it. They play a crucial role in reducing transepidermal water loss, strengthening the skin barrier, and protecting the skin from external irritants. Without adequate ceramides, the barrier cannot function properly.

Cholesterol 
Cholesterol accounts for approximately 25% of the lipids in the skin barrier and is essential for maintaining skin suppleness and comfort. When skin feels persistently tight or inflexible, it is often a sign that cholesterol levels within the barrier are depleted. It is important to clarify that topical cholesterol used in skincare is skin-identical and safe. It has no effect on blood cholesterol levels.

Fatty Acids
Fatty acids, both essential (such as linoleic acid) and non-essential, make up around 10–15% of the skin barrier lipids. They support the transport and organisation of ceramides and cholesterol within the barrier, provide nourishment, and help the skin retain moisture. Fatty acids are vital for maintaining barrier flexibility and supporting repair.

Supporting Lipids and Hydration Factors
The remaining 10–15% of the barrier lipid structure consists of supporting lipids and precursors that help maintain hydration and barrier functionality. These include triglycerides, phospholipids, and components of the skin’s Natural Moisturising Factors (NMFs), which work within the skin cells to attract and hold onto water.

Pro Tip
Some skincare products focus on ingredients that encourage the skin to produce its own lipids rather than supplying them directly. The difference is similar to growing your own ingredients versus buying them ready-made. While these supportive ingredients can be beneficial, particularly over the long term, the skin’s ability to produce vital lipids declines with age and compromised barrier health. This means they may not deliver immediate results when the barrier is already damaged. For clarity, I refer to these ingredients as the supporting cast of the skin barrier, rather than the main stars.

 

Barrier Support Ingredients (The Support Act):

Sphingolipids
Sphingolipids are naturally occurring lipids that form part of the skin’s barrier structure. They help support moisture retention, reinforce barrier strength, and contribute to overall skin resilience, particularly when the barrier is under stress.

Phytosterols (Plant Sterols)
Phytosterols are plant-derived compounds that are structurally similar to cholesterol. In skincare, they help reinforce the skin barrier, improve hydration, and reduce inflammation. They are particularly useful in barrier-supportive formulas where cholesterol itself is not used.

Niacinamide (Vitamin B3)
Niacinamide supports barrier function by encouraging ceramide production within the skin while also helping to calm redness and inflammation. It can be highly effective for strengthening the barrier over time, but some skins find it sensitising, especially when the barrier is already compromised, so it is best introduced with care.

Essential Fatty Acids
Essential fatty acids provide important structural support for the skin barrier and play a role in lipid organisation and resilience. While the skin barrier itself is largely composed of non-essential fatty acids, essential fatty acids are still crucial for overall barrier health and repair.

Humectants
Humectants attract and bind water within the skin, helping to keep it plump, hydrated, and comfortable. They also support the skin’s Natural Moisturising Factors (NMFs), which are vital for maintaining hydration within the skin cells and supporting barrier function.
